Use this form when requesting TeleBlock Service for additional switches or when you need to remove a switch from the TeleBlock service. (password required) Provide each of your Telemarketing customers a copy of this form when they sign up for TNS's TeleBlock service. Your telemarketers must complete this form to supply the information needed for their proper provisioning. (password required)
